This looks like a transparent hinged protective box with a hanging chain, typically used as a safety cover over an emergency stop push button or similar control.
It’s a transparent mini electrical panel lockout with chain, designed to cover and lock square push buttons or selector switches on panels where no inbuilt lockout provision exists.
Ideal locations: MCC panels, control desks, and machine panels where OEM did not provide lockable covers on emergency stop, start, or selector switches
Box-type cover with square inner cavity that encloses the projecting part of the switch; fitted with a metal chain so the device remains tethered to the panel and is not misplaced.

Two padlock holes accepting shackles up to approximately 6.8 mm, enabling single or multiple padlock use for group LOTO.
Product type and use
Used as part of an overall Lockout/Tagout procedure; one or more padlocks plus tags can be applied so only authorized personnel can remove the device and re-energize the circuit.
Operation: The base is fixed over/around the control point; when the cover is placed on, padlocks are applied through the integrated holes so the cover cannot be removed and the button cannot be pressed.
